Request Medical Records

 There are two easy ways to get a copy of your medical record. Choose option #1 or #2. 

1. Complete the Medical Record Release form found HERE and then bring into your current practice location.

2. Request records via ShareCare. Click the button below and complete each section of the Authorization for Disclosure of Health Information form.
