Request Medical Records

 There are two easy ways to get a copy of your medical record. Choose option #1 or #2. 

1. Request records via ShareCare HERE or click the button below and complete each section of the Authorization for Disclosure of Health Information form.

2. Complete the Medical Record Release form found HERE and then bring into your current practice location.
