What is Chronic Care Management (CCM)?

Managing a rheumatology condition can be challenging. Chronic Care Management (CCM) is a service to give you ongoing care outside of and in between doctor visits, so you don’t have to manage your health alone.

Who Qualifies?

You may be eligible for CCM service if you have two or more chronic conditions, examples include, but not limited to:

• Rheumatoid arthritis
• Osteoarthritis
• Lupus
• Psoriatic arthritis
• Gout
• Osteoporosis
• Or any other chronic conditions

Services must be provided under the general supervision of a provider. Please note only one provider is eligible for CCM services fee per calendar month.

How does CCM help you?

With CCM, you receive:

  • A personalized care plan created with your rheumatology team
  • Routinely monitors your progress with your goals and interventions
  • Medication and treatment adherence support to help improve outcomes
  • Coordinated care across your rheumatologist, primary care provider, and other specialists on patient’s behalf for referrals
  • Direct patient to educational resources, finding support groups, and more
  • 24/7 access to clinical staff for urgent health questions and health information
How does CCM help you?

CCM can help you:

  • Collaboratively develop health goals with your care team and interventions to achieve your goals
  • Frequent check ins and communication with care team, stay on track with medications, therapies, and lab work
  • Improve patient care by encouraging adherence to medications and treatment plans which can help reduce hospital visits and hospital readmissions
  • Access to care team in between your appointments
  • Manage referrals to other providers and scheduling appointments

What Does it Cost?

  • Medicare and most insurance plans cover CCM
  • Depending on your coverage, you may have a small monthly copay
